Another year has come to a close and with it the opportunity
to look back and reflect on where God has been active in the life of our churches.

We ask each CGGC church to complete the Ministry Review and return it to our office by April 15th, 2026.
We are making a push toward our online version of the form for faster and easier completion. With that in mind, we have region and eldership specific forms. Please click below for your region's form. 

We are working to finalize a Spanish version of the 2025 Ministry Review. Once uploaded, this message will be altered.

The digital survey is quick and easy.
Provided you use a Gmail account to fill out the form, you can even save your progress and finish it later! Otherwise, we recommend jotting down your responses in another place to make sure you don't lose anything.
